The Urban Indian Health Institute (UIHI) developed the Urban Diabetes Care and Outcomes Summary Report to provide a summary of the Indian Health Service (IHS) Diabetes Care and Outcomes Audit (Diabetes Audit) data collected by participating IHS funded Urban Indian Health Organizations (UIHOs). This report’s format is based on the IHS Diabetes Best Practice Guidelines. Included in the report are select key measures from the Best Practice Guidelines, with accompanying aggregated data from participating UIHOs. Three individual programs are also highlighted, to spotlight examples of some of the innovative work currently taking place around the country. 44 p.
