The Urban Indian Health Institute (UIHI) developed the Urban Diabetes Care and Outcomes Summary Report to provide a description of the Indian Health Service (IHS) Diabetes Care and Outcomes Audit (Diabetes Audit) data collected by participating IHS funded Urban Indian Health Organizations (UIHOs). This report’s format is based on the IHS Diabetes Best Practice Guidelines.
