Implementing a new case management tool: the diabetes patient care summary.

Although the utility of a flowsheet documenting annual flow-up needs and changes in certain parameters has been well established, the paper diabetes flowsheet is infrequently used by primary care providers. This article discusses the results of a project that developed an electronic version of the Resource and Patient Management System, which is used at many facilities serving American Indians. The project, called the Diabetes Patient Care Summary (DPCS) was successful.
